Study on the disordered eating behaviors in adolescents with type 1 diabetes and influencing factors

Objective To investigate the current situation of disordered eating behaviors in adolescents with type 1 diabetes mellitus,and to analyze the influencing factors of disordered eating behaviors. Methods A total of 187 adolescents with type 1 diabetes mellitus were recruited from a tertiary hospital during follow-up by the convenient sampling method from December 2021 to September 2022. The General Information Questionnaire,the Diabetes Eating Problem Survey-Revised,the Problem Areas in Diabetes Scale-5,the Diabetes Strengths and Resilience Measure for Adolescents,and the Dual-Mode of Self-Control Scale were used to explore the current status of disordered eating behaviors in adolescents with type 1 diabetes. We adopted the multiple linear regression model to analyze the influencing factors of disordered eating behaviors. Results 174 participants completed the questionnaires. The scores of disordered eating behaviors were(21.25±10.93) points. The incidence of disordered eating behaviors was 54.60%. The multiple linear regression analysis showed that the influencing factors of disordered eating behaviors involved gender,BMI,mental distress,resilience,and self-control. Conclusion High disordered eating behaviors exist in adolescents with type 1 diabetes. Medical practitioners should regularly evaluate patients’ disordered eating behaviors,especially female,obese or overweight patients. Through correcting the patients’ problematic body image cognition,and enhancing their confidence in disease control,medical staff could help them cope with stress positively,reduce negative emotional experiences,and further decrease the occurrence of disordered eating behaviors.

Evidence summary for non-pharmacological interventions of breast cancer-related lymphedema

Objective To integrate the best evidence of non-pharmacological interventions for breast cancer-related lymphedema with evidence-based methods,and to provide evidence-based bases for standardized evaluation and management of lymphedema for clinical medical staff. Methods The best evidence about the prevention and management of lymphedema in breast cancer patients were searched systematically,such as relevant guidelines,expert consensuses,systematic reviews and so on. The retrieval period was from January 2016 to January 2022. Results A total of 25 pieces of the literature were included,including 6 guidelines,4 expert consensuses and 15 systematic evaluations,and 33 pieces of best evidences were integrated from 2 aspects,namely the preventive intervention and therapeutic intervention. Conclusion The summarized evidence can be used for the prevention and treatment of breast cancer lymphedema,and medical staff should provide specific health guidance for patients with lymphedema based on cultural background,clinical situation,and patients’ opinions.

Effects of structured individual cognitive stimulation therapy on patients with mild to moderate dementia:a meta-analysis

Objective To evaluate the effect of structured individual cognitive stimulation therapy in patients with mild to moderate dementia. Methods Pubmed,Embase,Web of science,Cochrane library,CINAHL,APA PsycInfo and CNKI,Wanfang Database,CBM were searched by computers for the research on the efficacy of structured individual cognitive stimulation therapy for patients with dementia from inception to September of 2022. 2 researchers conducted literature screening,data extraction and quality evaluation independently,and meta-analysis was performed by RevMan 5.4 software. Results A total of 9 pieces of literature were included,in which 617 patients with mild to moderate dementia were studied. The results of meta-analysis showed that the test group was better than the control group in Mini-Mental State Examination scale scores[MD=1.15,95%CI(0.34,1.96),P=0.005] and Montreal Cognitive Assessment scale scores[MD=4.88,95%CI(2.16,7.60),P<0.001],but there was no significant difference in ADAS-cognitive portion scale scores[MD=-1.20,95%CI(-3.02,0.62),P=0.200]. The improvement on depression in the test group is better than that of control group[MD=-0.81,95%CI(-1.31,-0.31),P=0.001].However,there is no statistically significant differences in the quality of life[MD=1.65, 95%CI(-0.58, 3.88), P=0.150) and neuropsychological symptoms[MD=-0.23,95%CI(-2.77,2.32),P=0.860] between 2 groups. Conclusion The existing evidence shows that the structured individual cognitive stimulation therapy could alleviate the depression of patients with mild to moderate dementia,and improve the cognitive ability to a certain extent,but the effect on improving the quality of life and neuropsychological symptoms is not evident,which needs to be further studied with high-quality randomized controlled trials in a larger sample size.

Development of a knowledge-attitude-practice assessment questionnaire for the investigation of home bathing status of ostomy patients and the test of its reliability and validity

Objective To develop a knowledge-attitude-practice assessment questionnaire for home bathing status of ostomy patients and to test its reliability and validity. Methods Based on the theory of knowledge,attitude and practice,the initial questionnaire was generated through literature review and group meetings. The final questionnaire was formulated by 2 rounds of Delphi expert consultations and pilot-testing. A convenience sample of 386 stoma patients who visited the stoma outpatient clinic of a tertiary hospital in Zhejiang Province from July to December 2022 were selected to examine the reliability and validity of the questionnaire. Results The final knowledge-attitude-practice assessment questionnaire included 3 dimensions of knowledge,attitude and behavior,with a total of 24 items. The CVI of each item in the questionnaire ranged from 0.800 to 1.000,with an average of 0.932. Exploratory factor analysis showed that cumulative contribution rate was 67.181%. Confirmatory factor analysis showed that the assessment questionnaire had a good fit:the chi-square degree of freedom ratio(χ2/df)was 1.510;the fitting goodness index(GFI) was 0.878;the comparative goodness of fit index(CFI)was 0.948;the root means square error of approximation(RMSEA) was 0.049. The total Cronbach’s α coefficient of the questionnaire was 0.922,and the Cronbach’s alpha coefficient of each dimension was 0.920,0.862,0.897;the test-retest reliability coefficient of each dimension was 0.856,0.942,0.926. Conclusion The knowledge-attitude-practice assessment questionnaire has good reliability and validity,and it can be used as a valid tool for medical staff to carry out assessment and education of home bathing for patients with stoma.

Evidence summary for prevention and management of the skin adverse reactions in patients with targeted tumor therapy

Objective To select and summary the best evidence on prevention and management of the skin adverse reactions in patients with targeted tumor therapy,and to provide references for clinical decision. Methods We systematically searched for the evidence on prevention and management of the skin adverse reactions in patients with targeted tumor therapy in BMJ best practice,UpToDate clinical decision system,Joanna Briggs Institute,Guidelines International Network,National Institute for Health and Clinical Excellence,Scottish Intercollegiate Guidelines Network,Yi Maitong Guidelines Network,National Guideline Clearinghouse,the Agency for Healthcare Research and Quality,Registered Nurses’ Association of Ontario,World Oncology Network,National Comprehensive Cancer Network,Lung Cancer Committee of China Anti-Cancer Association,Chinese Anti-Cancer Association Oncologic Nursing Comittee,Cochrane Library,Embase,CHINAL,Web of Science,OVID,PubMed,SinoMed,CNKI,WanFang database,VIP database to collect the literature,including clinical guidelines,evidence summaries,best practice information sheets,recommended practice,systematic reviews and original research. The retrieval period was from January 1,2011 to October 3,2021.There were 2 researchers evaluating the quality of the literature and extracting the data. Results 15 articles were selected,including 3 guidelines,4 evidence summaries,1 systematic reviews,3 randomized controlled trials,1 class experiment and 3 expert consensuses. 19 pieces of best evidence were formed from 6 aspects,including skin evaluation,risk factors,drug therapy,skin care,skin adverse reaction management and health education. Conclusion This study summarizes the best evidence on management and prevention of skin adverse reactions in patients with targeted tumor therapy,and provides evidence-based evidence for clinical workers and managers. It is suggested that clinical staff should combine clinical context,fully assess patient’s skin condition,do a good job of skin health education,examination and care to prevent the occurrence of skin adverse reactions.

Construction and preliminary application of Symptom Assessment Scale for Donors of Allogeneic-Hematopoietic Stem Cell Transplantation

Objective To develop a Symptom Assessment Scale for Donors of Allogeneic-Hematopoietic Stem Cell Transplantation(Allo-HSCT) and its reliability and validity were tested. Methods Based on the literature reviews and semi-structured interviews,the item pool of the scale was developed. The initial scale was formed through 2 rounds of Delphi consultations. Through the pre-survey of 30 donors,the descriptive definition of each item and the reference standard of symptom severity was clarified,and the clinical test scale was formed. A questionnaire survey was conducted on 178 Allo-HSCT donors who admitted to the departments of hematology in 5 tertiary hospitals in Zhejiang Province from January to December 2021 to test the reliability and validity of the scale. Results The Symptom Assessment Scale for Donors of Allo-HSCT included 21 items,and the exploratory factor analysis extracted 7 common factors,and the cumulative variance contribution rate was 67.794%. The content validity index of each item was 0.830~1.000,and the content validity index of the scale was 0.884. Cronbach’s α coefficient,retest reliability and half reliability were 0.811,0.755 and 0.730,respectively. Conclusion The Symptom Assessment Scale for Donors of Allo-HSCT developed in this study has good reliability and validity,and it can be used for symptom assessment of related population.

Nursing care of patients with deep ulcer of extremities after cobra injury repaired with skin flaps

This article summarized the perioperative nursing experience of 21 patients with free anterolateral femoral perforating flap in repairing deep ulcer of limbs caused by cobra injury. Nursing key points included:before surgery,adequate wound preparation,ultrasound positioning of blood vessels in the donor site,and effective closed drainage with negative pressure,and psychological care;during surgery,pay attention to temperature monitoring and blood pressure control,accurately cooperate for anastomosis of arteries and veins,and prevention of intraoperative arterial crisis;after surgery,pay attention to nursing care of skin flaps,observation of blood supply of flaps,prevention and timely management of vascular crisis of flaps,regulated usage of sensitive antibiotics during all procedures,and early rehabilitation training. There were no necrosis or partial necrosis of the skin flap in 21 patients.Perioperative precise nursing is an important guarantee for the success of free anterolateral femoral perforating flap in repairing the deep ulcer of limbs caused by cobra injury.
